Febrile Seizure


Febrile seizure
Definition
A febrile seizure is a convulsion in young children that may be caused by a sudden spike in body temperature, often from an infection. Watching your child experience a febrile seizure can be alarming. And, although a febrile seizure may last only a few minutes, it may seem like an eternity to you.

Fortunately, febrile seizures aren't as dangerous as they may look. A seizure triggered by a fever is usually harmless and typically doesn't indicate a long-term or ongoing problem.
You may feel helpless during a febrile seizure, but you can make sure your child is safe during a seizure and offer comfort afterward. Following up with a doctor's visit is another good approach for handling a febrile seizure.

Symptom
Febrile seizure symptoms can range from mild — rolling of the eyes — to more severe shaking or tightening of the muscles.
  • A child having a febrile seizure may:
  • Have a fever usually higher than 102 F (38.9 C)
  • Lose consciousness
  • Shake or jerk the arms and legs on both sides of the body
  • Roll his or her eyes back in the head
  • Have trouble breathing
Febrile seizures are classified as simple or complex:
  • Simple febrile seizures. These are the most common type of febrile seizure, and they can last from a few seconds to 10 minutes and stop on their own.
  • Complex febrile seizures. A complex febrile seizure lasts longer than 15 minutes, occurs more than once within 24 hours or is confined to one side of your child's body.
Although a febrile seizure is usually caused by a rapid rise in your child's temperature, it can also occur when the fever is on its way down. And, the severity of the signs and symptoms doesn't necessarily reflect the level of the fever.

When to see doctor 
Have a first-time febrile seizure evaluated by your doctor as soon as possible, even if it lasts only a few seconds. If the seizure ends quickly, call your doctor as soon as it's over and ask when and where your child can be examined. If the seizure lasts longer than five minutes or is accompanied by vomiting, a stiff neck, problems with breathing or extreme sleepiness, call for an ambulance to take your child to the emergency room.

Causes
Most febrile seizures occur because of a sudden spike in body temperature, and most occur during the first day of a fever. But a febrile seizure may also develop as the fever is declining.

Viral or bacterial infection
Usually, the fevers that trigger febrile seizures are caused by an infection in your child's body. Typical childhood illnesses, including viral infections such as roseola — a viral illness that causes swollen lymph nodes, usually in the neck, and a rash — are often associated with febrile seizures. A less common but very serious cause of sudden fever with seizures is an infection of a child's brain and spinal cord (central nervous system), such as meningitis or encephalitis.

Post-immunization seizures
The risk of febrile seizures may increase after some childhood immunizations. Febrile seizures may rarely occur eight to 14 days after a measles-mumps-rubella (MMR) vaccination. Low-grade fevers can sometimes occur after your child receives childhood vaccines. If a febrile seizure occurs, it's caused by the fever that may accompany the vaccination — not by the vaccination itself.

Risk factors
Several risk factors have been identified that increase your child's likelihood of experiencing a febrile seizure. These include:
  • Age. Young age is the strongest risk factor, and most febrile seizures are seen in children who are between 6 months and 5 years of age. It's unusual for children under 6 months old to have a febrile seizure, and it's rare for these seizures to occur after 3 years of age. About 1 in 25 children experience a febrile seizure.
  • Family history. Some children inherit a family's tendency to have seizures with a fever. Additionally, researchers have linked several genes to a susceptibility to febrile seizures.

Complications
Although febrile seizures may cause great fear and concern for parents, most febrile seizures produce no lasting effects. Simple febrile seizures don't cause brain damage, mental retardation or learning disabilities, and they don't mean your child has a more serious underlying disorder.

Febrile seizures also aren't an indication of epilepsy, a tendency to have recurrent seizures caused by abnormal electrical signals in the brain. The odds that your child will develop epilepsy after a febrile seizure are small. Only a small percentage of children who have a febrile seizure go on to develop epilepsy, but not because of the febrile seizures.

Recurrent febrile seizures

The most common complication of febrile seizures is the possibility of more febrile seizures. About a third of children who have a febrile seizure will have another one with a subsequent fever.

The risk of recurrence is higher if:
  • Your child had a low fever at the time of the first febrile seizure.
  • The period between the start of the fever and the seizure was short.
  • An immediate family member has a history of febrile seizures.
  • Your child was younger than 15 months old at the time of the first febrile seizure.

Preparing for your appointment
You're likely to start by first seeing your child's family doctor or pediatrician. However, you may then be referred to a doctor who specializes in disorders of the brain and nervous system (neurologist).

Because appointments can be brief, and there's often a lot of ground to cover, it's a good idea to arrive well prepared. Here's some information to help you get ready for your appointment, and what to expect from your child's doctor.


What you can do
  • Write down everything you remember from your child's seizure, including any signs or symptoms that occurred before the seizure, such as a fever.
  • Make a list of all medications, vitamins and supplements that your child takes, if any.
  • Write down questions to ask your doctor.

Preparing a list of questions can help you make the most of the time with your child's doctor. 
List your questions from most important to least important in case time runs out. For febrile seizures, some basic questions to ask your doctor include:
  1. What's the most likely cause of my child's seizure?
  2. What kinds of tests does my child need? Do these tests require any special preparation?
  3. Will this happen again?
  4. Does my child need treatment?
  5. What do I need to do the next time he or she has a fever?
  6. What can I do to help my child if he or she has another febrile seizure?
  7. My child has another health condition. How can we manage them together?
  8. Are there any brochures or other printed material that I can take home with me? What Web sites do you recommend visiting?
  9. Will giving my child fever-reducing medications during an illness help prevent febrile seizures?

In addition to the questions that you've prepared to ask your doctor, don't hesitate to ask questions during your appointment at any time that you don't understand something.

What to expect from your doctor
Your doctor is likely to ask you a number of questions about your child's seizure, such as:
  • Did your child have a fever or illness before having this seizure?
  • Can you describe your child's seizure? What were the signs and symptoms? How long did the seizure last?
  • Has this ever happened before?
  • Does anyone in your child's family have a history of febrile seizures or seizure disorders?
  • Do you know if your child has been exposed to any illnesses?
  • Does your child have any history of head trauma or a neurological disease?

What you can do in the meantime
Here's what you can do if your child experiences another febrile seizure:
  1. Don't restrain your child, but do place him or her on a safe surface, such as the floor.
  2. Place your child on his or her side, keeping the face to the side and the lower arm extended under the head, to prevent your child from inhaling vomit if vomiting occurs.
  3. If your child had anything in his or her mouth when the seizure began, remove it. And, don't place anything in your child's mouth during a seizure because doing so can cause choking.
  4. If a seizure lasts longer than five minutes, seek emergency care.

Tests and diagnosisAfter experiencing a febrile seizure, your child will likely have:
  • A physical exam
  • Blood tests
  • Urine tests

These tests can help determine possible causes of the fever and seizure.

If your doctor suspects a central nervous system infection, a lumbar puncture (spinal tap) may be necessary. In this procedure, a doctor inserts a needle into your child's lower back to remove a small amount of spinal fluid. This test can reveal evidence of infection in the fluid that surrounds the brain and spinal cord.

Further tests such as an electroencephalogram (EEG) — a test that measures brain activity — may be necessary if your child had a complex febrile seizure.

Treatments and drugs
It's not necessary to lower your child's fever to stop a febrile seizure. So don't try to give your child fever medications during a seizure. For the same reason, don't place your child in a cooling tub of water. It's much more practical, more comfortable — and safer — for your child to remain lying on the carpet or a bed.

Most febrile seizures stop on their own within a couple of minutes. If your child has a febrile seizure that lasts more than five minutes — or if your child has repeated seizures — call for emergency medical attention.

More-serious episodes
In rare cases, the seizure may continue until your child receives treatment at the emergency room. If this happens, a doctor may order medication that's administered either through your child's rectum or intravenously to stop the seizure.

If the seizure is prolonged or accompanied by a serious infection or if the source of the infection can't be determined, your doctor may want your child to stay in the hospital for further observation. But a hospital stay isn't routinely necessary for simple febrile seizures.

Lifestyle and home remedies
If your child has a febrile seizure, stay calm and follow these steps to help your child during the seizure:
  • Place your child on his or her side, somewhere where he or she won't fall.
  • Stay close to watch and comfort your child.
  • Remove any hard or sharp objects near your child.
  • Loosen any tight or restrictive clothing.
  • Don't restrain your child or interfere with your child's movements.
  • Don't attempt to put anything in your child's mouth.

If possible, try to time the seizure using your watch or a clock. Because they're so alarming, seizures often seem to last longer than they really do. Also try to note which part of your child's body begins to shake first, and look for other signs of illness. This can help your doctor understand the cause of the seizure.

Not long after having a febrile seizure, many children are back on their feet, running around the doctor's office or playing safely at home. By staying calm, observing your child and knowing when to call the doctor, you're doing everything that's needed to take care of your child.

Prevention
Most of the time, a febrile seizure occurs the first day of an illness. Often, a febrile seizure occurs before parents realize that their child is ill.

Giving your child medications
Giving your child acetaminophen (Tylenol, others) or ibuprofen (Advil, Motrin, others) at the first indication of fever will help reduce the fever, but won't necessarily prevent a seizure. And, there are some caveats to giving medications to young children. Medications won't shorten the course of infection, and low-grade fevers generally don't need treatment.

Additionally, there's always a question of safety when giving medications to young children. For example, aspirin may trigger a rare but potentially fatal disorder known as Reye's syndrome. And, while acetaminophen is generally safe, if you give a child too much, it can cause liver failure. Ask your doctor what medications he or she recommends and what the proper dosage is for your child's age and weight.

Making sure your child drinks plenty of fluids and is appropriately dressed — not overdressed — may help control the fever.

Prescription prevention medications
Rarely, prescription medications are used to prevent febrile seizures. Anticonvulsant medications such as phenobarbital, valproic acid (Depakene) and divalproex sodium (Depakote) can prevent febrile seizures when taken daily. Oral or rectal diazepam (Valium, Diastat) also can reduce the risk of febrile seizures if taken at the time of a fever.

But these medications all have drawbacks. They carry a definite risk of serious side effects in young children, such as sleep problems, irritability and hyperactivity. Doctors rarely prescribe these prevention medications because most febrile seizures are harmless and most children outgrow them without any problems.

story etc are good..
although ends abruptly .. i mean not too abruptly..
but if you kill the final villain without letting him come through ..
its a cheating.. :D

Valkyrie (2008)


Valkyrie means Chooser of the Slain. According to Norse Mythology, the Valkyrie choose who will die in the battle and bring them to the hall of Slain i.e. Valhalla in front of the god Odin. They also serve the slain heroes as their lovers.

The movie Valkyrie is a historical thriller set in Germany during World War II and is based on real incidents. There had been 15 known incidents of attacks on Hitler, the Fuhrer of Germany. Valkyrie depicts one of them, the 20 July 1944 plot by German army officers to assassinate Adolf Hitler using Operation Valkyrie which was actually a national emergency plan to take control of the country. Valkyrie was a reserve force of about 10,000 army personnels (if I remember correctly from the movie) which once effective could arrest even the politicians and take over the country e.g. in case a coup is suspected.

Directed by Bryan Singer (X-men, Superman Returns), Valkyrie starred Tom Cruise as Colonel Stauffenberg, one of the key plotters of this incidence. Though, everybody likes movies where the hero finally takes over the villain and its a happy ending type of story. Valkyrie stands out in this case coz this is a story of a failed attempt.. but its a must watch nonetheless..

The other guys that I had seen before included Bill Nighy (Viktor from Underword series) and Tom Wilkinson (RocknRolla, awesome performance as Lenny Cole).

A-Team

 

This was a movie which turned out to be a very good entertainer and that too completely unexpectedly.. We were served a good amount of action and comedy in a fantastic manner.. Whereas Liam Nesson rocks as he mostly does, the other guys were also pretty good.. especially Bradley Cooper, who turned out to be quite awesome.. Jessica Beil had a supporting actress type role.. i mean I can't even mention her ..

Anyway, you would certainly enjoy this movie if you haven't already.. :D

Shaurya 2006


Shaurya is one of the best movies Bollywood has produced.. I have heard praise for K.K. Mennon from almost everybody. He has been a great actor in Bollywood .. and he definitely has done justice to the role of Jack Nicholson in A few Good Men. Our Tom Cruise is not so young, though he also is a very good actor.. After a real long time did I see JAved Jaffrey in a good and serious role.. Although its a remake of A few Good Men, its not like the other Bollywood remakes in not being a trash.  ;)

A Few Good Men (1992)


A few Good Men is definitely one the best movies of Tom Cruise and Jack Nicholson. The topic is on a few US Army people doing whatever they think is right in the name of discipline. Its an awesome movie to watch. Although, while watching it again recently I was left with a few questions like, why wasn't the effect of death of MArkinson (the key witness) on Tom Cruise shown a little more drmatically. And why would an army officer be taking his court martial so lightly that he would re-enact his foolish strong image in the court room, while initially he tried his smartness.

Another hindi (Bollywood) movie called Shourya, was a remake of A few Good Men, and that on is also quite interesting. Nonethelss, go ahead and do watch the movie, leave comments if you please.. :D

Prince 2010


And here comes another Bollywood flick which talkes the same thing as Paycheck did.. Taurani's have spent enormous amount of money ..This is the first movie where I liked all the dresses of almost everybody. :D
Locations have been chosen very carefully and if somebody hasn't watched Dhoom 2 or Paycheck he/she may like the locations and the dresses.. However, coming to the main point i.e. the story and performances, the movie drops only shit and does nothing much.. Atif Aslam's song can't really support the movie .. Vivek Oberoi doesn't do well here too..

Paycheck is directed by John Woo who happens to be a very good action movie director. He has also directed some unforgettable movies like, Mission Impossible: II and Face-Off. Although, paycheck is a good action movie, it definitely isn't John Woo's best creation, however, saying this certainly undermines the reality of the film. Judging by the time of about 2003 when 3-D world sleeping in its deep recesses and might not have been thought to rise and shine till late 20s or so, I think he deserves an applause for making a nice action movie..

Gordos (Spanish)



Awesome.. a must watch..
Gordos means fat in spanish, so, Gordos is a story about some fat people. It tells about the miserable lives that many fat people have but it doesn't stop there. The characters start to fight their own nature, one that brings feelings of being pathetic. Their lives turn almost 180 degree creating huge voids. This has been depicted very well in the movie ..
Additionally, the so-called perfect life that all of us look for, has also been shown in its true shade.

Although, the movie contains a lot of nudity and fat people having sex, I might not be biased in calling it an elegant film with amazingly good direction by Daniel Sánchez Arévalo. Right now, I can only smile as my salute to the director who not only picked a sensitive subject but did enormous justice to it too...
But decide yourselves..

The movie is just too good to miss..
It's been more than about 5-6 months when I saw it.. but the smile of night fury when our hero brought her some fish is unforgettable.. :D

With Adam Sandler, one thing is for sure that you can die laughing. Its not just a comedy, its a very good, light and sexy comedy. Now that I have seen many of his movies I find him a really good actor, totally worth watching. If you wanna laugh for about an hour or so, do watch this movie..

Aakrosh



Enjoy Aakrosh..
Priyadarshan has taken a nice job to propagate some good social message to the masses. Aakrosh is about honour killings occurring widely in INDIA, plus, how the "dalits" have been suppressed by higher classes.

Ajay Devgan and Bipasha Basu are very good in their roles, Akhshaya Khanna is good but somehow his acting is not so genuine or serious .. but then he has been so for so long.. Isn't it?
The "hool aur Kante" type action has been repeated in a car chase scene but its not even original now.. and doesn't even leave an impact.. Probably, the action director has tried to introduce "Free Rnning" sequence for the first time in bollywood as I don't remember specifically which movie featured free running before.

All in all, enjoy a nice movie..

The Millenium Trilogy


Amazingly captivating.. 
Anybody interested in a fight for crime against women would find it really engrossing.
This series of movies depicts the fight of a young girl (not so young, though) with the anti-women elements of society. The heroine, Lisbeth Salander, is the daughter of a brutal Russian terrorist (smuggler of both women and arms to Sweden). She is mentally tortured by his father's brutality against her mother. One day she puts his father on fire.. and a never ending story starts for Lisbeth Salander, fraught of pain and perils.

The name of the series comes after a magazine, Millenium, of which the hero is a publisher. And the three movies are viz. The Girl with Dragon Tattoo, The Girl Who played with Fire and The Girl who kicked Hornet's Nest. The novels on which the movies are based on were written by Stieg Larsson.

Badmaash Company


I didn't know it was directed by "Parmeet Sethi". Most of us would know him as "Kuljit" of DDLJ or even better as husband of Archana Pooran Singh. He has done an adorable job in his directorial debut. Thankfully, most of the scenes are not melodramatic and there was always a crookedness in the behavior of all the four friends/actors.
Shahid Kapoor's acting looked OK but not exceptional as he was in "Kaminey"..
Anushka Sharma looked a little better than her debut film "Rab Ne Bana Di Jodi", though she still lacks the oomph! Chang, one of the final contenders of "Indian Idol 3", is better than what is expected from beginners and so is Vir Das.

Aisha



Aisha was a good movie .. Abhay Deol is different as always and Sonam Kapoor really looked cute (but sometimes too huge!!).
I liked it's light-hearted and less burdened story...
It reminded me of "Monsoon Wedding" .. both films share almost same style of film-making.

At its later stages it became a drag but even then it was a clean and clear kind of movie .. being better than many others

The Last Airbender

With a film as technologically advanced as Avatar, the expectations of realistically throwing balls of fire and water were pushed to the limit on The Last Airbender.

I seriously believe that this could have been a masterpiece, taking 20/10 or at least 10/10, but M. Night Shyamalan stays just a bit behind the mark.. Well, to me the story was good, the costume design and characters were all good, the graphics were brilliant and flawless (unless i missed sthg; i was truly delighted by many of the graphically enhanced scenes; watch out for the Dragon) but the story was little too jumpy and abrupt manyatimes. Initially, for the first half I almost felt like leaving it in between but I somehow continued to the end and was delighted by the climax.

There are four nations each with an element (air, fire, water and earth) that they can control. The FIRE nation has grown stronger and wants to expand its power and control further. But the Avatar (who can control all the four elements) is their main hurdle.

just remember that it has Mallika Sherawat with awesome body and that too served nude, just for you (ahem.. i mean your eyes only)...

The best scenes were her Holi dance and when she was hit on her shoulder and Irfan Khan takes her out of a rope-web..

Lamhaa (2010)



"GOD made heaven, Man traded it"

Sitting away from the most beautiful valley of Kashmir I can never be a judge of why can't some of the people come out of a perpetual self-destructive loop they are in. How people have misled others, how the INDIAN army got involved to make things right using the best way known to them, taking a lot of flak in it and how did it backfire? or then did it actually backfire?

There are many questions in my mind as of now..

Set in Kashmir, (the most beautiful and dangerous place in the world?) the film Lamhaa is a story about an army officer, played by Sanjay Dutt (as stiff as always) , and his love interest (Bipasha Basu; as pretty and sexy as always), in the midst of the various issues that Kashmir had faced over the years. You will find Anupam Kher and Kunal Kapoor also in supporting roles (both of which are really good actors; though they don;t get much to show their real might).


The song "Salaam Zindagi" was highly emotional..
And i liked it when Sanjay Dutt says once "Kashmir Bilkul nahin badala, Aaj bhi Jannat hai.."

Imagine a movie where the great-oldies Mithun Chakravorthy, Shatrughan Sinha and Dharmendra are heroes without any story to play. I would certainly not watch the movie except if somebody told me that it features really hot heroines like Jessica Alba, Michelle Rodriguez (Fast and Furious) and Lindsay Lohan (who was nude almost all the time). So now that you have a feel of the movie. checkout the heroines. Michelle Rodriguez shows the best figure in this one.. especially at the end.. just like the showstoppers of a fashion show... :D

Ninja Assassin

Great action ...
For all the action lovers, it's a must watch.
The stunts have been performed well, and it's a treat..

Though, all the whole movie goes really well until the story shows some disrespect for the whole concept its based on..i.e. as the main theme of the movie is that Ninjas being unbeatable assassins and more so when its dark.. so Ninjas prefer dark almost all the time. But once when the hero is taking heat from other ninjas they become so desperate that come out open on the road (in the light). Except for that bogus scene, rest of the movie is awesome..

Have you ever heard of Guy Ritchie?..
When I first heard about him after Madonna married him I thought why would Madonna marry a guy whom I don't even know? heehehe ee .. . I realized the minisculity of my brain when I saw "Sherlock Holmes".

And now it is RocknRolla...
When the movie starts you know from the numbering itself that when different kind of guys work together the outcome is different .. and I enjoyed the outcome though it was not as fantastic as Sherlock Holmes. It was good..It also starred Gerard Butler.. and sexy Thandie Newton (remember MI:2 beauty).
For some reason I enjoy English movies more than American movies..

Law Abiding Citizen

I first saw Gerard Butler in a History Channel series on the Huns called-- "Atila-The Hun". He was awesome in that series.. I was thoroughly impressed by his performance and then i saw him in a few films after that.. most notable if which was "300". He rocked in 300 as well.. but as a typical hero he just doesn't fit.. No doubt he is good.. but he fits more in aggression than normal up and down feeling.

 Law abiding citizen is also good..
It tells the story of a normal law abiding citizen whose family is murdered in front of his eyes..
and the killers can still escape the death penalty.. free to commit other crimes..
Gerard is not happy the way the case was dealt with by his attorney, one of the best in town..
So, he goes into the books and smarts up himself enough to give the Law guys a real tough time..
A completely unexpected end which was a bit too abrupt.. and quick.

He is the Shooter, the best that US has, who after a govt-op-gone-bad incident looses his friend and exiles himself to some place high up in mountains. But he has got one last job to finish..

Good.. and how do we say it.. Fantabulous?
Yup this is fantabulous.. after watching so many thrillers, action, and drama this movie is a change.
I remember when  I saw Step Up 2: The Streets which was a real good feeling..
every minute of its dancing moves made my heart beat faster..

You must listen to the song  Beggin' by Madcon its awesome

Although, Street Dance 3D is a little milder that Step Up 2 but it's fun nonetheless..

Resident Evil: Afterlife

This is among a few good series of sequels which are full of some good action.
The first three parts of Resident Evil series were quite impressive. This part of the RE series is based on the latest Capcom game "Resident Evil 5". Many of the characters have been borrowed from RE5, especially the villain Wesker and hero Chris. Now, whereas the games itself will make you go nuts and bow to the creativity of its developers this movie doesn't seem to deliver enough ..
However, unlike Dabangg which had only Salman to watch and nothing else.. RE.AL. has Mila Jovovich along with some concept and some story.. although it needed more explanation than what has been provided.

So, finallly, I could get "The Expendables" on a real good print ..
It is a blue-ray rip with A/V quality exceeding 9/9..


Steve Stonecold Austin (remember WWE; he was my favorite there) has been rarely used..
they could've used him somewhere strong..
Arnold, Bruce Willis have guest appearance only..But I liked Mickey Rourke's presence..
Jason Statham still rules my favorite list of actors in this movie and otherwise, especially when we talk adrenaline... ;) He delivers well .. he is good.. really good..
I was so confused when I watched Crank and then Crank 2: High Voltage..
I didn't know whether they were too good or too bad.. but it was way different..
Jet Li has been barely used.. So, basically, this movie is just like one amalgamation when you mix a lot of good, tasty things but the outcome is not tangy enough...

The Expendables also has a strong resemblance observable to another good movie "A-Team" launched earlier this year .. It, however, had high expectations and just about delivers them (if it would to many).. To me, after such a huge cast involving Sylvester Stallone, Jason Stathom, Jet Li, and Dolph Lundgren this movie should have delivered above 8/10.. But alas! it doesn't.. 
Anyway, enjoy it..

Khatta Meetha

First I thought that Priyadarshan has lost his sense of film-making..
But then there were times when I really laughed especially in the scene where Asarani is busy and Akshay Kumar goes to ask for some money for her sisters marriage.. .  :D
His capability to direct was really worth an applause during the end run/fight sequence. I liked the camera work a lot..

I Hate Luv Storys (2010)

Hey guys I liked it..
May be I had free time as I had just returned after watching hockey in the Commonwealth Games 2010 at New Delhi. or whatever, but the thing is .. I liked it... :D
Imran khan has done a good job and so has Sonam Kapoor..
Its all good.. do watch it.. coz a lot of times it was a good laugh ..
The most laughing character is the one who would imitate Shahrukh Khan ..
he is hilarious.
